In this role I am accountable for the operation of the fellowship training program. This includes establishing and maintaining an appropriate educational environment, supervising faculty and other program personnel at each participating site, preparing an accurate statistical and narrative description of the program for the ACGME and ensuring the implementation of fair policies, grievance procedures and due process within the training program. The goals and Objectives for this program were completely revised within a year of my arrival in 2007 to make the program compliant with the ACGME program guidelines that had been substantially upgraded that same year. The program has a five year accreditation cycle and is free of any citations.

Responsibilities include management of acute post operative and chronic pain patients. As the Director of the Pain Management Center, I provide oversight of the multidisciplinary team that includes three physicians, staff psychologists and physical therapists. The Pain Management Center has experienced significant growth during this period of time. The interventional pain management portion of the program grew from less then 100 flouroscopic guided procedures in 1998 to over a thousand procedures annually by 2007. The practice also experienced a 50% increase in new patient consultations in the last five years of my tenure.

Fellowship in Pain Management at the University of Rochester:

Role: Director October 15, 2007 to November 1, 2015

This is a 12-month ACGME approved clinical fellowship in pain medicine. As the program director I maintain and update the policy manual for pain management fellows, continuously revise curriculum, organize weekly didactic conferences and arranged outside speakers to maintain the educational environment and satisfy ACGME mandates regarding program requirements. I provided guidance and assistance to the fellows in the preparation of lectures for the medical students, nurses, and residents. I also have primary responsible for all elements of the fellowship’s training program and in completing the program information form reviewed by the ACGME to determine ongoing accreditation status.

Pain Medicine Fellowship at the University of Maryland:

Role: Faculty, January 1995-June 1998; Director June 1998 – September 2007

This is a 12-month ACGME approved clinical fellowship in pain medicine. As the program director I developed a policy manual for pain management fellows, continuously revised curriculum, organized weekly didactic conferences and arranged outside speakers to maintain the educational environment and satisfy ACGME mandates regarding program requirements. I provided guidance and assistance to the fellows in the preparation of lectures for the medical students, nurses, residents and in preparation of an annually required grand rounds presentation. Major modifications to the curriculum were completed in 2002 to incorporate the six general competencies described in the ACGME’s program guidelines. In 2006 new assessment tools were developed to provide the pain medicine fellows with 360 degree evaluation. This program is approved for three fellows per year and has consistently trained 2-3 physicians per year since 1998.

Senior resident rotation in pain management for CA-3 residents at the University of Maryland:

Role: Faculty, 1/1995 – 6/1998; Director 7/1998-September 2007

One-to two month clinical rotation: Senior anesthesiology residents participate in care of patients with complex pain management problems and those being managed with advanced pain management interventional techniques. Senior residents participate in readings, conferences and bedside teaching addressing the more complex pain management issues. The curriculum was revised in 2006 to reflect changes in the ACGME’s guidelines for the training of anesthesia residents in the field of pain medicine. Senior residents are assigned more challenging cases and increasing levels of clinical responsibility as compared to the junior residents. 12-14 residents rotate through this service annually.

Junior resident rotation in pain management for CA-2 residents at the University of Maryland:

Role: Faculty, 1/1995 – 6/1998; Director 7/1998 – September 2007

This is a month long rotation in pain management. Junior anesthesia residents are introduced to fundamental management concepts in the management of pain conditions in both inpatient and outpatient settings. The curriculum was revised in 2006 to reflect changes in the ACGME’s guidelines for the training of anesthesia residents in the field of pain medicine. The curriculum includes weekly conferences designed to teach the basics of pain management; problem based learning discussions, bedside teaching on the attending rounds and in the fluoroscopy suite. 12-14 residents rotate through this service per year.

Four-week course in pain management for medical students at the University of Maryland

Role: Director 1998- September 2007

Course includes didactic conferences, readings and hands on teaching addressing basics of assessment and management of pain in a comprehensive multidisciplinary setting. A written evaluation based on direct observation and feedback from residents, fellows and other pain faculty is prepared at the end of rotation.

I reviewed the Pain Management Chapter in the seventh edition of American Academy of Hospice and Palliative Care Medicine’s Primer of Palliative Care August 2018

I reviewed a lecture entitled “Review of Opioid Pharmacology”. This was reviewed on behalf of the on line publishing site Anesthesia Toolbox in August 2017.

I reviewed a manuscript entitled “Use of bilateral glossopharyngeal nerve neurolysis in a patient with cancer of the tongue base”. Reviewed for the journal Pain Physician June 2015.

Community Service:

6/1/2016- 7/1/2017: I served as a member to the Community Principles for Pain Management which is a specialty advisory group that provides resources to the New York State medical community at large regarding state of the art pain management. I lead the acute pain management subcommittee for this initiative.

I have served as an expert witness for both the State of Maryland and State of New York regarding cases of inappropriate opioid prescribing. I currently review 2-4 cases per year on behalf of the State of New, the majority of which regard concerns related to the oversight and appropriateness of opioid therapy.
